THE EFFECT OF PARATHYROID HORMONE ON LEFT VENTRICULAR FUNCTIONS IN CHRONIC HEAMODIALYSIS PATIENTS

Cardiovascular complications are importent mortality and morbidity reasons in chronic haemodialysis patients. It is thought that parathyroid hormone (PTH) leads to cardiac complications and parathyroidectomy or active D3 vitamin can reduce these complications. However, in some haemodialysis patients, PTH is within normal ranges but similar complications can be seen in these patients. In this study, we aimed to compare the effect of PTH on left ventricular functions in patients with normal and high levels of PTH. Thirteen patients (5 female, 8 male) with high levels of PTH and 12 patients (6 male, 6 female) with normal levels of PTH, all nonparathyroidectomised and not taken active D3 vitamin. In group with high PTH levels, mean PTH was 491.7i.124.4pg/mL, cardiac output (CO) 4.8±1.5L/min, peak E velocity 8014cm/sc, peak A velocity 8916cm/sec, E/A 0.94±0.38, mitral E deceleration time (DT) 110±l 5.8msec, isovolemic relaxation time (IVRT) 84.4±13.3msec, left ventricle mass index (LVMI) 118±19gr/m2 , relative wall thickness (RWT) 0.51±0.11. In group with normal levels of PTH, mean PTH was 54.3±14.8pg/mL, CO 4.4±1.4L/min, peak E velocity 78±18cm/sec, peak A velocity 84.2±21 cm/sec, E/A 1.1±0.21, DT 120±24.6msec, IVRT 89.5±17msec, LVMI 123±27gr/m2 , RWT 0.61 ±0.14. Left ventricular systolic and diastolic functions, LVMI and RWT were statistically indifferent in both groups. In conclusion, parathyroid hormone does not have any effect on left ventricular functions and geometry of chronic haemodialysis patients.
